Why take this test?

If you’ve been diagnosed with subclinical hypothyroidism this test will be able to tell you if you have an increased chance of developing an underactive thyroid. This test will also tell you if an underactive thyroid is likely caused by autoimmune disease.

What is subclinical hypothyroidism?

  • When a person’s thyroid tests show a raised level of TSH but a normal level of free T4.
  • People with subclinical hypothyroidism and raised TPO antibodies are more likely to develop an underactive thyroid than those with normal levels of TPO antibodies.
  • The term subclinical hypothyroidism only applies to people who have not been diagnosed with a thyroid condition. Subclinical hypothyroidism is also sometimes called mild hypothyroidism.

What are TPO antibodies?

  • TPO stands for Thyroid Peroxidase.
  • They are a protein found inside your thyroid gland.
  • High levels of TPO antibodies can indicate that someone may have an autoimmune disease affecting the thyroid.

What do my TPO antibody results mean?

A result of 35 or above:

You have a raised level of TPO antibodies. This suggests that you may have a higher chance of developing an underactive thyroid in the future. If you have already been diagnosed with an underactive thyroid, this suggests that this condition is caused by an autoimmune disease affecting the thyroid.

A result of 34 or below:

Your TPO antibody levels are normal. Your chances of developing an underactive thyroid are lower. If you’ve already been diagnosed with an underactive thyroid then this result does not indicate whether or not this was caused by an autoimmune disease.

“This test is most useful for people who have had thyroid function tests that were abnormal. The main time we would suggest having this test is if you have been specifically diagnosed with ‘subclinical hypothyroidism’ (when the thyroid results are only somewhat abnormal – the TSH is raised but T4 is normal). In these cases, having a positive TPO antibody result predicts that you are more likely to develop full blown hypothyroidism in the future, and you may benefit from closer monitoring if you are not taking thyroid medication.
